At Ethos Home Health Care and Hospice, it is important to us that your patients receive the care they need in a timely manner. We invite you to use any of the options below to place a referral to our home health care or hospice services.

Option 1: Use our secure online form

Option 2: Call or fax us

You can call or fax us using the appropriate number below. Download and fill out the home health care or hospice form for fax referrals.

Home health care services include - skilled nursing, physical, occupational, and speech therapy, home health aides, remote health monitoring, personal emergency response pendants, and more.

Hospice services include - skilled nursing, home health aides, spiritual care, social workers, life-enriching services, and more.

Option 3: Use Epic

If you're an Epic user, you can place a referral to Ethos using the in-basket feature by following the instructions below:

Step 1

Open the patient chart

Step 2

Select Care Management Navigator

Select DC Placement
- Click Home Care and Hospice
Utilizing the search bar, select add
- Search by name of Agency
- Select E Link option
Mark accept the on bottom right of the screen
New Communication Tab
Discharge Placement Report
Enter a small note with information to the agency
Select Send
- This will be kept in the patient's records.
- Epic facility can see when it was opened and when the agency read the in-basket message.
